2. When you receive the guide and the prices for the options we have selected for you, just decide on which levels of cover you are interested in. If you let us know your choice, we will then send you the relevant brochure, application form and hospital list.

3. To join a scheme you simply need to fill in the application form and return it to us in the freepost envelope provided.
